Accessing Your GP Health Record Online

Patients registered with the practice can access their GP health record online using the NHS App or by signing in through the NHS website at www.nhs.uk.

Online access allows you to view information held in your GP record, including:

  • Current and past medications
  • Appointments
  • Test results
  • Consultation notes
  • Letters and documents that have been added to your record

To access your record online, you must:

  • Be registered with our practice
  • Have an NHS login

Most patients are automatically able to see new information added to their GP record. Any information recorded after online access has been enabled will usually be available to view through the NHS App or NHS website.

Keeping your information safe

Your online health record contains confidential personal information. Please keep your NHS login details secure and do not share them with anyone else unless you want them to have access to your health information.

If you think someone may have accessed your account without your permission, please contact the practice as soon as possible.

Access to older records 

Online access may not include all historical information held by the practice. If you would like access to older records or require a full copy of your medical records, please contact the surgery directly. Requests for copies of medical records (Subject Access Requests) can be made verbally or in writing and will normally be processed within one month in accordance with data protection legislation.

Information that may not be available online

In some circumstances, certain information may not be visible online. This may be where access could cause serious harm to the physical or mental health of an individual, or where information relates to another person and should remain confidential.

Proxy access

Parents, guardians, carers and other authorised representatives may be able to request proxy access where appropriate. All requests are assessed individually and may require identity verification and evidence of authority to act on behalf of the patient.Please contact the practice for further information about eligibility and how to apply.

Keeping your information safe

Patients should be aware that choosing to share their NHS login details with another person may allow that individual access to confidential health information.  Where another person needs access to help manage healthcare, formal proxy access is recommended.
